A coalition consisting of home care vendors, accreditation programs and associations has partnered to provide a national study on reducing avoiding hospitalizations through home care.

The National Study on Best Practices to Reduce Unplanned Hospitalizations has two goals: to identify the best practices that are used by hospital-based and non hospital-based home care agencies to reduce unplanned hospitalizations; and, based on the findings, initiate a systematic effort that results in home care agencies not only learning about the best practice findings, but also adopting the findings.

This national study seeks to address an issue that has been named as a major goal of all five of the most significant initiatives in health care reform – Care Transitions, ACOs, Medical Homes, Bundling and Value Based Purchasing.
