The Agency for Health Care Administration has asked HCAF to notify all home health agencies that User Agreements need to be sent to the Home Care Unit as soon as possible and no later than December 15, 2012. If the User Agreements are not received and approved, home health agencies will not be able to access the web portal to do the Quarterly Report beginning January 1, 2013.
To obtain a User Agreement for the HHQR, every licensed home health agency will need to…
(1) Identify an authorized individual to register as the User who will access the new system and submit the quarterly report.
(2) Go to the web portal* and complete and send a HHQR User Agreement, signed by the administrator, for each individual user designated to access and submit the quarterly report online to the Agency.
3) Send the signed User Agreements either by fax to (850) 922-6059 or scan and e-mail with the subject line: “HHQR User Agreement”.
Please do not mail the User Agreements.
