FAU Partnering with HHAs to Reduce Rehospitalizations

fsuFlorida Atlantic University is seeking participation from Florida home health agencies to pilot test the INTERACT (Interventions to Reduce Acute Care Transfers) Quality Improvement Program for Home Health. INTERACT is a quality improvement program that focuses on the management of acute change in patient condition. It includes clinical and educational tools and strategies for use in every day practice. The pilot test will run for a 3-6 month period beginning in July 2013, and require participating agencies to:

  1. Implement the INTERACT Quality Improvement Program using the PAPER ONLY version of the draft Version 1.0 Tools for Home Health.
  2. Provide routine feedback to the project team (includes CMS members) via brief online surveys and brief telephone interviews.

The application requires HHA to select the Quality Improvement Program tools they want to test during the pilot. The tools are similar to the Version 3.0 tools for nursing homes which can be viewed at http://interact2.net.

Applications should be completed online by July 12, 2013. A separate application is needed for each home health provider requesting to become a pilot site. Click here to access the application for those interested in becoming a pilot site. Please complete this no later than July 12, 2013.
