FAU Partnering with HHAs to Reduce Rehospitalizations

Florida Atlantic University is seeking participation from Florida home health agencies to pilot test the INTERACT (Interventions to Reduce Acute Care Transfers) Quality Improvement Program for Home Health. INTERACT is a quality improvement program that focuses on the management of acute change in patient condition.
